Representation & Equality Monitoring Form

As part of our ongoing efforts to responsibly monitor, evaluate and improve on intersectional representation and equality within our workforce, we are asking all individuals we work with to fill in the below form. This is not an essential requirement, but all the data collected is used to help us highlight where we need to make improvements in ensuring a workforce that is representative of the Global Majority, and also to support any positive action programmes we may implement under the Equality Act 2010.

  • Your answers, and your data, are anonymous and securely stored. We do not ask for any contact details or identifiable information such as name, number or email address.

  • You are free to 'Prefer not to say' to any of the below questions.

  • We are constantly learning and evolving our understanding of representation and equality monitoring. If you feel the responses available to you in this form do not accurately reflect how you identify, or how others may identify, please let us know in the final comment box so we can ensure we educate ourselves and action accordingly.
